联合检测外周血癌胚抗原mRNA及livin mRNA对早期发现大肠癌微转移肿瘤的意义研究

Significance of Peripheral Blood CEA mRNA and livin mRNA in Combined Detection of Micro Metastases in Colorectal Cancer Patients

目的研究癌胚抗原(CEA)mRNA和livin mRNA在大肠癌患者外周血中的表达情况,探讨二者联合检测对早期发现大肠癌血道微转移肿瘤的价值。方法选取60例大肠癌患者、30例大肠良性疾病患者和30例健康体检正常者,反转录聚合酶链式扩增(RT-PCR)法检测3组CEA mRNA及livin mRNA在外周血中的表达情况。结果大肠癌患者中CEA mRNA阳性表达率为58.3%(35/60),livin mRNA阳性表达率为61.6%(37/60)。大肠良性疾病组和对照组中CEA mRNA和livin mRNA均无阳性表达。3组CEA mRNA和livin mRNA阳性表达率比较,差异均有统计学意义(P<0.05)。大肠癌患者外周血中CEA mRNA和livin mRNA的表达与性别、年龄、肿瘤分化程度及肿瘤部位无明显相关性(P>0.05),而与肿瘤的临床分期、有无淋巴结和远处转移有关(P<0.05)。大肠癌患者CEA mRNA和livin mRNA联合检测阳性率为86.7%(52/60),与CEA mRNA单项阳性率58.3%(35/60)及livin mRNA单项阳性率61.6%(37/60)比较,差异有统计学意义(P<0.05)。结论 CEA mRNA和livin mRNA联合检测能够提高大肠癌血道微转移肿瘤的阳性检出率,可以用于预测大肠癌患者的预后。

Objective To investigate the expression of CEA mRNA and livin mRNA in peripheral blood of colorectal cancer（CRC） patients,to evaluate their significance in discovery of Cancer Micro metastases.Methods The expressions of CEA mRNA and livin mRNA in peripheral blood were detected by reverse transcription polymerase chain reaction（RT-PCR） in 60 CRC patients（CRC group）,30 with intestinal benign diseases（IBD group） and 30 healthy subjects（control group）.Results The positive rate of CEA mRNA and livin mRNA were 58.3% and 61.6% in CRC group,but no positive expressions in groups IBD and control.The difference was significant（P0.05）.The expressions of CEA mRNA and livin mRNA were not significantly correlated with gender,age,tumor differentiation and location（P0.05）,but positively with clinical staging of tumors,lymph node,distant metastasis（P0.05）.In CRC patients,the detection positive rate of combined CEA mRNA and livin mRNA was 86.7%（52/60）,that of single CEA mRNA was 58.3%（35/60）,that of livin mRNA was 61.6%（37/60）,the difference was significant（P0.05）.Conclusion Detection of combined CEA mRNA and livin mRNA,which improves the positive detection of CRC micromeatastasis,can be used in prognosis prediction of CRC patients.
