引起泌尿道感染肠球菌的调查浅析

摘要 目的:了解肠球菌在泌尿道感染中的分布特性,分析肠球菌的耐药现状,为临床的治疗及抗生素的合理使用提供依据。方法:对我院在2009年6月~2010年7月期间各科室的中段尿送检标本中分离的210株肠球菌进行鉴定和对10种抗生素的药敏试验;药敏试验采用琼脂纸片扩散法(K-B法)。结果:210株肠球菌的鉴定以粪肠球菌(53.3%)和屎肠球菌(44.2%)最多,其他肠球菌只占2.5%;药敏试验中,粪肠球菌和屎肠球菌对除万古霉素以外的其他抗生素具有较高的耐药性,并且两者的耐药性存在差异;粪肠球菌HLGR(高水平耐氨基糖苷类抗生素肠球菌)分离率为88.5%,屎肠球菌HLGR分离率为85.9%;粪肠球菌VRE(耐万古霉素肠球菌)分离率2.8%,屎肠球菌的VRE分离率为1.1%。结论:肠球菌是泌尿道感染的重要致病菌,仅次于大肠埃希氏菌;粪肠球菌的分离率高于屎肠球菌,而屎肠球菌的耐药率高于粪肠球菌;HLGR的检出率较高,而VRE检出率低;万古霉素仍是治疗肠球菌引起的泌尿道感染的最有效药物。 Objective : Enterococci in urinary tract infection to understand the distribution characteristics of drug resistance of enterococci for clinical treatment and provide the basis for the rational use of antibiotics. Methods: Our hospital in June 2009 - July 2010 during the inspection of an sections of the urine samples of 210 strains of enterococei isolated were identified and antibiotic susceptibility test 10 ; susceptibility testing by agar disk diffusion method (K - B method). Results:Identification of 210 isolates of enterococci to Enterococeus faecalis (53.3%) and feces Enterococcus (44.2%) the most, only 2.5% of other enterocoeci; susceptibility test, Enterococcus faecalis, and feces than than vancomycin the other has a high resistance to antibiotics, and differences between the two drug resistance ; Enterococcus faecalis HLGR (high level resistance to aminoglycoside antibiotics in Entcrococcus faecium) isolated was 88.5 % ,feces HLGR isolation rate was 85.9% Enterococcus;Enterococcus faecalis VRE (vancomycin resistant enterococci) isolation rate of 2.8 %, feces Enterococcus VRE isolation rate of 1.1%. Conclusions: Enterococci are important pathogens in urinary tract infections, second only to Eschericbia coli;Enterocoecus faecalis and Enterococcus isolated higher in feces, and feces is higher than the resistance of Enterocoecus faecalis;HLGR detection higher, while the low rate of VRE detection;vancomyein remains the treatment of urinary tract infections caused by enterococei the most effective drugs.
