摘要
目的探讨HLA-DRB1、-DQA1和-DQB1等位基因多态性与中国藏族人群肺结核易感性之间的关系。方法采用聚合酶链反应/序列特异性引物(PCR/SSP)技术对176例中国西藏地区藏族肺结核患者和189例该地区藏族健康人群的HLA-DRB1、-DQA1和-DQB1等位基因多态性进行分析。结果 HLA-DRB1*0901、-DQA1*0101和-DQB1*0301在肺结核组的等位基因频率明显高于健康对照组,2组相比差异具有统计学意义(17.33%vs 7.94%,χ2=17.185,P=0.000,OR=2.811;25.85%vs 13.76%,χ2=22.382,P=0.000,OR=2.821;35.51%vs 19.58%,χ2=37.329,P=0.000,OR=3.809)。HLA-DRB1*1301/1302、-DQA1*0501在肺结核组的等位基因频率明显低于健康对照组,2组相比差异具有统计学意义(0.85%vs 6.08%,χ2=15.085,P=0.000,OR=0.125;14.49%vs 26.72%,χ2=22.44,P=0.000,OR=0.355)。结论 HLA-DRB1*0901、-DQA1*0101和-DQB1*0301与肺结核的易感性密切相关,可能是肺结核的易感基因。HLA-DQA1*0301与肺结核的保护性密切相关,可能为肺结核的保护基因。
Objective To study the correlation between polymorphisms of HLA-DRB1,-DQA1 and-DQB1 alleles and susceptibility to pulmonary tuberculosis in Tibetan population of China.Methods HLA-DRB1,-DQA1 and-DQB1 alleles in 176 patients with pulmonary tuberculosis and 189 normal controls were analyzed using the polymerase chain reaction/sequence specific primer(PCR/SSP) technique.Results The frequencies of HLA-DRB1 * 0901,-DQA1 * 0101 and-DQB1 * 0301 alleles were significantly higher in pulmonary tuberculosis patients than in normal controls(17.33% vs 7.94%,25.85% vs 13.76%,35.51% vs 19.58%,Chi-square=17.185,P=0.000,OR=2.811;Chi-square=22.382,P=0.000,OR=2.821;Chi-square=37.329,P=0.000,OR=3.809),while the frequencies of HLA-DRB1 * 1301/1302 and-DQA1 * 0501 alleles were significantly lower in pulmonary tuberculosis patients than in normal controls(0.85% vs 6.08%,14.49% vs 26.72%,Chi-square=15.085,P=0.000,OR=0.125;Chi-square=22.44,P=0.000,OR=0.355).Conclusion HLA-DRB1 * 0901,-DQA1 * 0101 and-DQB1 * 0301 alleles are closely related with the susceptibility to pulmonary tuberculosis and may be the susceptible genes against pulmonary tuberculosis.HLA-DRB1 * 1301/1302 and-DQA1 * 0501 alleles are closely related with the protection of pulmonary tuberculosis and may be the protective genes against pulmonary tuberculosis.
