高强度聚焦超声治疗乳糜尿近期临床疗效观察

Clinical observation on HIFU for the treatment of chyluria

目的探讨高强度聚焦超声（HIFU）治疗乳糜尿安全性和有效性。方法采用FEP-BY02型高强度聚焦超声肿瘤治疗机,对65例乳糜尿患者进行治疗。采用多层面,由深层到浅层的方法,对患侧肾蒂及输尿管上段水平组及垂直组淋巴管进行聚焦超声治疗。结果治疗后术1～4个月常规尿乳糜定性试验,转阴性49例（75.4%）,仍呈阳性16例（24.6%）。治疗后随访1年,49例显效患者中,乳糜尿消失率为87.8%。在治疗过程中,除术区轻度及中度疼痛外,未出现其它的并发症。所有患者治疗后3~12个月复查肾图、肾功能、尿液分析等均正常。结论高强度聚焦超声治疗乳糜尿具有无创伤、安全、有效、可反复治疗等特点。

Objective To evaluate the clinical effect and safety of High Intensity Focused ultrasound（HIFU） for treating the chyluria.Methods The 65 cases with this disease were treated by FEP-BY02 type high-energy focal tumor therapy-apparatus.Then the effect after heat radiation being adopted in the sick lateral kidney pedicle and upper ureter were observed.Results Regular lab-test for chyluria was taken on later 1-4 months after HIFU.Among the 65 cases 49 returned negative,the one-year follow-up showed the disappearance rate of the chyluria was 87.8%;16 were still positive.No complication occurred in the process of treatment with HIFU in the 65 cases.Pos-ttreatment 3-12 months,the renogra,renal function and urinalysis were normal.Conclusion HIFU is of reliable and effective method for the treatment of chyluria.
