周剂量紫杉醇同步后程加速超分割放疗治疗93例食管癌的临床研究

Study on Late Course Accelerated Fractionated Irradiation Combined with Concurrent Chemotherapy for 93 Cases Esophagus Squamous Cell Carcinoma

目的:观察放射治疗配合紫杉醇化疗对食管癌的临床疗效。方法:对173例食管癌病人根据不同的治疗方法分为两组:单纯放射治疗组(单放组)和放射治疗同步化疗组(放化疗组)。单放组80例,采用常规设野放疗,DT65-70 Gy;放化疗组93例,放疗方法及剂量同单放组,化疗采用紫杉醇每周同步方案。两组均随访2年。结果:局部近期疗效放化疗组好于单放组,两组有显著性差异(P<0.05)。放化疗组与单放组1、2年生存率单放组为51.3%(41/80)、32.5%(26/80),同步放化疗组为87.19%(81/93)、68.8%(64/93),两组差异有显著性(P<0.05)。结论:同步放化疗优于单纯放射治疗,可明显提高食管癌的疗效。

Objective：To study the effect of radiotherapy combined with paclitaxel chemotherapy on esophageal cancer.Method：173 cases of esophageal cancer were divided into conventional radiotherapy（80 cases） to a total of 65-70 Gy;combined group with received the same radiotherapy as simple irradiation group plus chemotherapy with weekly paclitaxel.Both groups were followed up for 2 years.Result：The short-term results of radiotherapy combined with chemotherapy was better than that of radiotherapy alone.The difference between these two groups was statistically（P〈0.05）.The one and two years survival rate were 87%,68.8% in combined.survival rate were 51.3%,32.5% in simple irradiation group.The survival rates in the combined group were thus much better than those in simple irradiation group（P〈0.05）.Conclusion：Radiotherapy combined with chemotherapy for advanced esophageal cancer is effective and merits further study in the clinic.
