彩超在恶性肿瘤患者颈内静脉置管后血栓形成中的诊断价值

The value of color Doppler flow imanging in diagnosing thrombosis after internal jugular vein indwelling intubation in patients with malignant tumor

目的探讨彩超在恶性肿瘤患者颈内静脉置管后血栓形成中的诊断价值。方法回顾130例颈内静脉置管后的恶性肿瘤患者的彩超诊断和纤维蛋白原(Fib)、血小板(Plt)、凝血酶原时间(PT)三个出凝血指标,分析颈内静脉内血栓的部位及大小演变。按静脉内是否有血栓将患者分成血栓组与对照组,并比较两组的凝血指标。结果 130例肿瘤患者中,86例(66%)颈内静脉内形成中等回声的团块样血栓;血栓位于静脉内的前壁、后壁、静脉瓣处,共有96处血栓。不同部位血栓的演变差异无统计学意义。血栓组与对照组间三个凝血参数相比较差异无统计学意义。结论彩超在恶性肿瘤患者颈内静脉置管后血栓形成的诊断中有重要价值。

Objective To evaluate the significance of color Doppler flow imanging（CDFI） in diagnosing thrombosis after indwelling catheter in the internal jugular vein in the patients with malignant tumor.Methods Data of CDFI and blood coagulation criteria in 130 cases with malignant tumor were retrospectively analyzed.The patients were divided into two groups of A（86 cases,with thrombus in the internal jugular vein） and B（44 cases,without）,between which,fibrinogen（Fib）,prothrombin time（PT） and platelet（Plt） were compared and variations of the location and size of the thrombus were analyzed.Results The incidence of venous thrombosis was 66%.There were 96 blood clots sticked to the front and rear wall of the internal jugular vein or on the valves.The differences of Fib,PT and Plt were not significant between groups of A and B.So did the changes of the thrombus among different locations.Conclusion CDFI has an important role in diagnosing thrombosis after indwelling catheter in the internal jugular vein in the patients with malignant tumor.
