针刀治疗不同证型腰椎间盘突出症的疗效观察

Curative effect observation of acupotomy therapy on lumbar disc protrusion of different type of syndrome

目的观察针刀治疗不同证型腰椎间盘突出症的疗效差异。方法对297例应用针刀疗法治疗的腰椎间盘突出症患者进行回顾分析,其中血瘀证107例,寒湿证63例,湿热证42例,肝肾亏虚证85例,应用视觉模拟评分法(VAS)、JOA下腰痛评分法评价针刀对不同证型患者的镇痛效果和临床疗效。结果针刀对各种证型腰椎间盘突出症的总有效率为100%,优良率为94.3%,血瘀证组、寒湿证组和湿热证组间的疗效比较无显著性差异(P均>0.05),而肝肾亏虚证组与其他3组间的疗效比较有显著性差异(P均<0.05)。结论针刀治疗腰椎间盘突出症疗效满意,且最适合血瘀证、寒湿证及湿热证等实证型。

Objective It is to observe the differences on curative effects of acupotomy therapy on lumbar disc protrusion of different type of syndrome.Methods Two hundred and ninty-seven cases of patients with lumbar disc protrusion treated with acupotomy therapy were analyzed retrospectively,including 107 cases of blood stasis,63 cases of cold and dampness,42 cases of dampness and heat,85 cases of deficiency of liver and kidney.The analgesic effect and clinical effect were quantified using VAS scores and the JOA scores after the treatment for different types of syndrome.Results The therapeutic effect of acupotomy evaluated by the JOA scores was a total effective rate of 100%,a good and excellent rate of 94.3%.According to comparison of effect among groups,the difference among syndromes of blood stasis,cold and dampness,dampness and heat was not significant（P0.05）,while there were marked differences between syndrome of deficiency of liver and kidney with other three groups（P0.05）.Conclusion Acupotomy therapy has a good curative effect on lumbar disc protrusion,especially for those excess syndromes such as syndromes of blood stasis,cold and dampness,dampness and heat.
