异丙酚对失血性休克复苏兔胃黏膜钙离子的影响 被引量:1

摘要 目的观察异丙酚对失血性休克复苏兔胃黏膜损伤及Ca^2+的影响。方法将成年新西兰雄性大白兔100只随机分为对照组(s)、模型组(HR—GMI)及失血前(P1)、复苏前(P2)和复苏后(P3)应用异丙酚组,每组20只。建立失血性休克复苏胃黏膜损伤(HR—GMI)模型。P1、P2及P3组分别于失血前、复苏前10min及复苏20min时静注异丙酚5mg/kg后,以20mg/(kg·h)持续泵注,对照组和HR—GMI组给予等量生理盐水。观察胃黏膜组织学改变,计算其病理损伤积分,检测胃黏膜Ca^2+浓度。结果①与对照组比较,模型组胃黏膜损伤严重,病理损伤积分和Ca^2+浓度明显升高(P均〈0.01)。②与模型组比较,应用异丙酚各组胃黏膜损伤明显减轻,病理损伤积分亦明显降低,且P1和P=2组Ca^2+浓度明显降低(P〈0.05或P〈0.01)。③与P3组比较,P1和P2组胃黏膜损伤较轻,且P1组的胃黏膜病理损伤积分和Ca^2+浓度明显降低(P均〈0.05)。结论异丙酚可降低失血性休克复苏兔胃黏膜组织内Ca^2+浓度,减轻胃黏膜损伤,失血前应用效果更佳。 Objective To study the effects of propofol on gastric mucosal injury and Ca2. after the resuscitation of hemorrhagic shock in rabbits. Methods One hundred New Zealand healthy adult male rabbits were randomly divided into 5 groups of 20 rabbits : control, model, pre - isehemia ( P1 ), pre - resuscitation ( P2 ) and post - resuscitation ( P3 ) injected propofol group. The hemorrhage resuscitationgastric mucosal injury( HR - GMI)model was established. In group P1, P2 and P3, 5 mg/kg propofol was injected intravenously 10 rain before ischemia, resuscitation and 20 min after resuscitation, then 20 rag/( kg. h) propofol was infused. In control and model groups, propofol was replaced with equal volume of normal saline. The gastric mucosal histological changes were examined and the pathologic lesion scores were calculated. The contents of Ca^2+ in the gastric mucosa were measured. Results (!)The gastric mucosa was injured seriously, and the pathologic lesion scores and the Ca^2+ content were increased significantly in model group compared with those in control group( all P 〈0. 01 ). (2)The gastric mueosal injury was relieved significantly and the pathologic lesion scores were decreased in all propofol groups and the Ca^2+ content were decreased significantly in group P1 and P2 compared with those in model group(P 〈0. 05 or P 〈0. 01 ). (3)The gastric mucosal injury was alleviated in group PI and P2, and the pathologic lesion scores and the Ca^2+ content were decreased significantly in group P1 compared with those in group P3 ( P 〈 0. 05 ). Conclusion Propofol can prevent the gastric mucosal injury caused by hemorrhagic shock resuscitation by depressing the Ca^2+ content in the gastric mucosa, and it is most effective that propofol was applied before isehemia
