靶控输注瑞芬太尼在腭咽成形术麻醉中的应用 被引量:3

摘要 目的通过观察瑞芬太尼靶控输注在腭咽成形术麻醉中的诱导插管期、维持期及恢复期的血流动力学变化和术毕停药后麻醉恢复时间,为临床合理用药提供参考。方法将40例行腭咽成形术患者随机分为两组:瑞芬太尼组(n=20)和芬太尼组(n=20)。诱导开始时给予静脉靶控输注(TCI)瑞芬太尼或芬太尼,设定其靶浓度为4 ng/mL,1 min后TCI丙泊酚,设定靶浓度为3μg/mL,观察全麻诱导插管期、维持期及恢复期的血流动力学变化,并记录术毕停药后睁眼时间和拔管时间。结果瑞芬太尼组与芬太尼组患者的睁眼时间分别为(4.2±1.2)min和(10.1±3.9)min;拔管时间分别为(9.7±1.4)min和(15.6±1.9)min,两组比较,差异有统计学意义(P<0.01)。两组患者全麻诱导后血压均有不同程度下降,均未经处理,插管后恢复正常,术中平均动脉压(MAP)总体有下降趋势,组间比较,差异无统计学意义。结论瑞芬太尼靶控输注用于腭咽成形术麻醉安全可靠。
