双膝前交叉韧带同期重建临床研究

Clinical Research of Simultaneous Bilateral Anterior Cruciate Ligament Reconstruction

目的:研究双侧膝关节前交叉韧带断裂后同期重建的临床疗效和安全性。方法:1995年1月至2008年6月,关节镜下行双膝前交叉韧带同期重建手术的41名患者纳入研究。围手术期观察止血带时间和手术并发症。所有患者平均随访6.7年(至少2年),随访内容包括膝关节IKDC功能评分、Lysholm评分以及KT-2000膝关节稳定性检查。将上述观察内容和与双膝组匹配的单膝重建对照组进行比较。结果:双膝组每侧止血带时间平均为59分钟,术后IKDC评分平均为86.7,Lysholm评分平均为92.4,膝关节前向稳定性的改善程度平均为6.0 mm;单膝组上述指标分别为63分钟、88.3、92.1和5.8 mm,随访结果显示两组无显著差异。结论:对于双侧膝关节前交叉韧带断裂病例,行同期重建手术安全、有效。

Objective To evaluate safety and clinical results after bilateral ACL reconstruction at a single setting.Methods From Jan 1995 to Jun 2008,41 patients who underwent simultaneous bilateral ACL reconstruction in our institute were retrospectively analyzed.As comparison,patients who underwent unilateral ACL reconstruction during the same time period were chosen as controls.Mean tourniquet time per knee and complications were recorded.All patients received evaluation at a mean follow-up of 6.7 years（at least 2.0 years）.The outcomes of both groups were evaluated by IKDC knee score,Lysholm score and KT-2000 arthrometry measures.Results The mean tourniquet time,IKDC knee score,Lysholm score and the improved knee stability（shown by KT-2000） were 59 minutes,86.7,92.4 and 6.0 mm per knee respectively for the bilateral group compared to 63 minutes,88.3,92.1 and 5.8 mm respectively for the unilateral group.There were no significant differences between the two groups at latest follow-up.Conclusion For patient with bilateral ACL deficient knees,simultaneous reconstruction of both knees was safe and effective.
