慢性腰背痛患者的脑静息态功能磁共振成像 被引量:14

摘要 目的观察慢性腰背痛患者静息状态下的脑功能活动特点。方法对15例慢性腰背痛患者及16名健康对照组受试者进行静息态脑功能磁共振成像,应用局部一致性(ReHo)方法处理fMRI数据,观察脑功能成像的差异。结果与健康对照组相比,慢性腰背痛患者双侧额叶、右侧枕叶、双侧前扣带回等脑区的ReHo值升高;中脑导水管周围灰质、臂旁核、脑桥、左侧顶叶后部、双侧扣带回后部及邻近楔前叶的ReHo值减低。结论静息状态下慢性腰背痛患者多个脑区的功能活动存在异常,可能与机体对持续疼痛刺激的适应性增加、内源性镇痛系统活动减低等有关。 Objective To investigate the characteristics of resting state brain functional activity in patients with chronic lumbodorsal pain.Methods Fifteen patients with chronic lumbodorsal pain and 16 healthy volunteers(control group) underwent brain functional MRI in resting state.The regional homogeneity(ReHo) approach was applied for analysis of fMRI data.The difference of brain fMRI between patients and healthy volunteers was studied.Results Compared to control group,patients with chronic lumbodorsal pain in resting state had increased ReHo in the bilateral frontal cortex,right occipital cortex,bilateral anterior cingulate cortex,as well as decreased ReHo in the periaqueductal gray,parabrachial nuclei,pons,left inferior parietal lobule,bilateral posterior cingulate cortex and precuneus.Conclusion Abnormal function activity of some encephalic regions exists in patients with chronic lumbodorsal pain in resting state,which may be related to the increased adaptability to persistent pain and decreased endogenic activity of relieve pain.
