Changes of T Lymphocytes Subsets in Patients with Acute Stroke

摘要 目的:探讨急性脑卒中患者T淋巴细胞亚群的变化。方法:连续收集发病24 h内入院的急性脑卒中患者161例为卒中组,入院时对其进行NIHSS评分;并以同时期门诊体检中心42例健康中老年人作为对照组。采用流式细胞仪测受试者入院第1天T淋巴细胞亚群变化,并进行相关性分析。结果:卒中组早期T淋巴细胞亚群CD3+、CD4+、CD8+较对照组显著下降(P<0.01);不同卒中类型(脑梗死与脑出血)T淋巴细胞亚群无显著差异。左侧大脑半球较右侧大脑半球受损患者CD3+、CD4+、CD8+ T淋巴细胞均减少;入院NIHSS评分与CD3+(r=-0.170,P=0.048)、CD4+(r=-0.286,P=0.001)T淋巴细胞表达呈负相关。结论:急性脑卒中患者发病早期即存在细胞免疫功能低下,病变性质不是决定性因素,不同卒中部位及损伤严重程度起着一定的作用。 Objective: To investigate the changes of T cell subsets in patients with acute stroke. Methods: One hundred and sixty-one patients with acute strokes admitted to the hospital within 24 h after symptoms onset were enrolled. The NIHSS scores were obtained after admission. Forty-two healthy aged people in the outpatient service medical center were recruited as controls. T lymphocytes subsets were measured by cell cycle analysis at day 1 after hospitaliza- tion. Results. The CD3+, CD4+, CD8+ expressions were decreased in acute stroke patients when compared with the controls(P〈0.01), but there was no difference between ischemic and hemorrhagic strokes. The CD3+, CD4+, CD8+ levels were decreased in acute stroke patients with infarction in the left cerebral hemisphere compared with those with infarction in the right cerebral hemisphere. There was a negative correlation between NIHSS and CD3+ and CD4+ levels. Conclusion. T lymphocytes immune functions were declined in early period of acute stroke patients. The sites of cerebral lesion and damage severity play an important role in the expression of T lymphocytes .
