摘要
目的检测环氧化酶-2(COX-2)、细胞周期蛋白D1(Cyclin D1)和磷酸化细胞外信号调节激酶1/2(p-ERK1/2在食管鳞状细胞癌(ESCC)、食管上皮内瘤变及正常黏膜组织中的表达,探讨其在ESCC发生、发展过程中的作用及其临床病理意义。方法采用免疫组化法检测90例ESCC、21例食管上皮内瘤变及69例正常黏膜组织中COX-2、CyclinD1和p-ERK1/2的表达情况。结果 COX-2、Cyclin D1和p-ERK1/2在食管正常黏膜、上皮内瘤变和ESCC组织中的阳性率分别为20.3%、28.6%、42.2%,1.4%、38.1%、63.3%和5.8%、14.3%、18.9%,其中Cyclin D1在3种组织间的表达差异有显著性(P<0.05),COX-2和p-ERK1/2的表达在ESCC和食管正常黏膜间差异有显著性(P<0.05)。COX-2的阳性率随癌组织分化程度的降低而增加,分别为28.6%、34.4%和68.2%,Ⅰ-Ⅱ级ESCC与Ⅲ级ESCC组之间COX-2表达差异具有显著性(P<0.05)。而Cyclin D1的阳性率随癌组织分化程度的升高而增加,分别为40.9%、70.5%和71.4%,Ⅰ-Ⅱ级ESCC组织与Ⅲ级ESCC组织之间Cyclin D1的表达差异具有显著性(P<0.05)。p-ERK1/2的表达与肿瘤的浸润深度、患者是否喜好热烫饮食(T≥70℃)及患者的5年生存率有关(P<0.05)。结论 COX-2、Cyclin D1和p-ERK1/2的表达与ESCC的发生、发展和预后具有一定的相关性,其高表达可能促进肿瘤的发生、发展。
Objective To investigate the differential expression of cyclooxygenase 2 ( COX-2), Cyclin D1 and 1: ERK1/2 in malignant, premalignant and normal esophageal tissues and its clinicopathological significance. Method Immunohistochemical staining for COX-2, Cyclin D1 and p-ERK1/2 was performed in 90 cases of esophageal squamous cell carcinoma (ESCC) tissues, 69 cases of normal tissues, and 21 cases of premalignant esophageal tissues with the tissue microarray. Results Positive rate of COX-2 in ESCC (42.2%) was significantly higher tha that in normal tissues (20.3%) (P 〈0.05), and was related to the tumor differentiation with 28.6%, 34. 4% 68.2% in the well, moderately and poorly differentiated tissues respectively, with the statistical significance (P 0. 05) between the well-moderately and the poorly differentiation tissues. Positive rate of Cyclin D1 in ESCC (63.3%) was significantly higher than that in premalignant (38.1%) and normal tissues (1.4%) (P 〈0. 01) and was related to the tumor differentiation with 40.9%, 70.5%, 71.4% in the poorly, moderately and well dii ferentiated tissues respectively, with the statistical significance ( P 〈 0. 05 ) between the well-moderately and th poorly differentiation tissues. Positive rate of p-ERK1/2 in ESCC ( 18.9% ) was higher than that in normal tissue (5.8%) (P 〈0. 05), and was related to the infiltration of tumor (P 〈0. 05 ), prognosis (P 〈0. 05) and hot drinking (T≥70℃) (P 〈 0.05). Conclusion Expression of COX-2, Cyclin D1 and p-ERK1/2 are closely relat ed to the formation, development and prognosis of ESCC.
