PFNA-Ⅱ治疗老年股骨转子间骨折 被引量：3

摘要 目的探讨PFNA-Ⅱ(proximal femoral nail antirotation-Ⅱ)治疗老年股骨粗隆间骨折的临床疗效。方法对63例老年股骨粗隆间骨折患者46例行PFNA-Ⅱ内固定,17例行DHS内固定,比较两组的手术时间、术中出血量、骨折愈合时间、功能评分结果。结果所有患者获得6～16个月随访,骨折全部愈合,PFNA-Ⅱ组平均手术时间49分钟,DHS组为87分钟,两者比较有显著性差异(P<0.001),术中出血量分别为130ml和580ml(P<0.001),愈合时间为12.8周和12.3周(P>0.05),功能评分优良率分别为82.6%和82.4%(按Harris评分标准)(P>0.05),PFNA-Ⅱ组出现髋内翻1例,异位骨化1例,未出现髓内钉、螺旋刀片钉断裂以及刀片切出股骨颈并发症。DHS组发生髋内翻1例,退钉2例,未出现侧板、主钉断裂以及主钉切出股骨颈颈的并发症。结论 PFNA-Ⅱ治疗股骨粗隆问骨折具有手术时间短、术中失血少、骨折固定牢固、并发症少的伏点,适合老年患者。
