摘要
目的探讨改良法超声引导下注射凝血酶治疗股动脉假性动脉瘤的价值。资料和方法 49例股动脉假性动脉瘤患者于超声引导下穿刺注射凝血酶,穿刺时压迫瘤颈使瘤内血流消失,瘤内血栓形成后逐级放松压迫力度直至瘤内填满血栓。分析假性动脉瘤的体积、分型、颈部长度宽度、凝血酶用量等对疗效的影响,观察其并发症。结果 49例患者术前经彩色多普勒超声确诊,其中单纯型37例,复杂型12例,动脉瘤平均体积为10.5±6.0cm3,瘤颈部宽度2.8±0.6mm,颈部长度6.1±1.4mm,平均凝血酶用量277±120U。46例一次成功,3例第一次注射后复发,经第二次注射后成功,其中单纯型1例,复杂型2例。未发生动脉或静脉栓塞等并发症。结论改良法超声引导下注射凝血酶治疗股动脉假性动脉瘤是一种高效、安全的方法,并发症少,可作为治疗的首选方法。
Purpose To evaluate the clinical application of improved ultrasound-guided percutaneous thrombin injection for the treatment of femoral artery pseudoan-eurysms(PA). Materials and Methods 49 patients with PA were treated with improved ultrasound-guided thrombin injection. Before injection, the PA neck was compressed so that no blood flow was inside. The size and type of the PA, length and width of PA neck, thrombin dose, therapy outcome and complication were analysed. Results All patients were diagnosed by color Doppler before injection. 36 were simple PA, and 13 were complex PA. mean volume of PA was 10.5±6.0cm3 .mean width and length of PA was 2.8±0.6mm and 6.1±1.4mm, respectively. Mean thrombin dose was 277±120U. The success rate of the first injection was 93.88%. 3 patients had PA recurrence after the first injection, but were cured after the second injection. No thromboembolic complications or infections occurred. Conclusion Improved ultrasound-guided thrombin injection enables successful and safe treatment of femoral artery pseudoaneurysms.
