合并糖尿病的颌面颈部间隙感染临床研究

Clinical Study of Maxillofacial and Cervical Space Infections with Diabetes

目的探讨颌面颈部间隙感染患者血糖偏高的临床特点及治疗要点。方法对近几年入住我科的56例颌面颈部间隙感染患者伴有糖尿病的临床资料进行回顾性分析。结果 56例患者均行手术切开引流,其中10例患者伴有严重的并发症,整个治疗期间严格监测调整血糖,全程足量使用抗生素,除1例伴有明显呼吸困难,准备气管切开,患者家属放弃抢救死亡外均痊愈出院。结论颌面部间隙感染的患者合并血糖升高,感染不易控制,容易引发严重并发症,病程延长,治疗应控制感染和降糖并重,采取综合治疗,可取得满意疗效。

Objective To investigate the clinical feature and treatment of maxillofacial and cervical space infections with diabetes. Methods 56 patients with diabetes in the maxillofacial neck clearance infection who came to our hospital in the recent years （Jan. 2006 -Jan. 2011 ） were analyzed retrospectively. Results 56 patients all adopted surgical incision and drainage, including 10 patients with severe complications followed by the strict monitoring adjustment during treatment and the entire enough blood glucose antibiotics, all mortality. Only one patient among them suffered from obvious difficulty in breathing, who had been offered to be treated with trachea cut and eventually died because of the refusal to be treated. Conclusion Diabetes mellitus and maxillofacial infection found in the patients are not easy to control, repeated attacks, duration extension, essential treatment should be applied in order to control the aspects of infection and blood sugar. If comprehensive treatment adopted, satisfying results can follow.
