摘要
目的:应用选择性血管介入治疗股骨头缺血坏死,评估其治疗效果。方法:选择股骨头缺血坏死患者53例,采用选择股骨头供血动脉3级分支插管,较大剂量药物灌注扩张血管、辅以关节外骨骼肌松解术治疗。用VAS疼痛评分表进行测分评价,49例随访6个月至3年,平均17.6月。采取自身对照方法,用VAS评价治疗前后症状改善,影象学变化。结果:32/49患者治疗后临床症状均有改善,患肢活动范围改善和无痛行走距离增加,14例髋部疼痛完全消失。X线分析,19例骨密度增加;CT发现17例囊性变周围出现骨钙化;骨扫描发现14例患侧同位素浓集下降。结论:选择性大剂量药物灌注辅以骨骼肌松解术治疗股骨头缺血坏死有较好疗效。
Objective： To investigate the effect of Interventional Therapy for Avascular Necrosis of Femoral Head.Methods： fifty-three patients with Avascular Necrosis of Femoral Head in Ficat I-II stage diagnosed by MRI or CT were performed Interventional Treatment.The medicine for dissolving the thrombus were used through catheterizing medial and lateral circumfles arteries that is Superselective blood supply vessels for femoral heads.The clinical symptoms and X-ray were followed up before and after therapy.Results： Hip pain and disorders of joint function are alleviated and improved in different degrees after treatment with catheterizing medial and lateral femoral head for 32 patientsas compared with that before treatment.With following-up one year,radiographs shows repair of the femoral head in different degrees for 19 patients.Conclusion： Interventional therapy improves blood supply and promots repair of the Avascular Necrosis of femoral head.
