浙江省农村结核病人获新农合补偿情况分析

Reimbursement of new rural CMS to rural TB patients in Zhejiang province

目的:了解浙江省各县区农村肺结核病人获得各地新农合政策对结核病人的补偿机制与报销情况,为浙江省结核病防治筹资与保障模式提供参考。方法:采用调查表对浙江省90个县区的新农合结核病补偿模式及报销情况进行调查。结果:全省90个县区中,共有84和43个县区的新农合对住院和门诊结核病进行补偿;一般结核病人在县级医院住院的中间档报销的平均比例为47.1%,门诊平均报销比例为25.1%;2008年平均每位肺结核住院病例能得到新农合补偿1 893元,门诊为163元。结论:浙江省农村结核病人在各县区新农合政策中能得到一定补偿,但补偿机制有待完善,补偿比例特别门诊的补偿比例有待提升,应将新农合作为结核病防治下一个十年规划的重要筹资模式。

Objective To understand the mechanism and status of new rural cooperative medical system（NCMS） reimbursement to TB patients at county-level in Zhejiang Province,and to provide suggestions to the fund raising and policy support of TB prevention and control.Methods With a fixed questionnaire,we collected the data of reimbursement system and status of NCMS in 90 counties of Zhejiang Province.Results In all 90 counties,there were 84 and 43 counties where NCMS permit to reimburse costs for TB inpatients and outpatients.The method was mostly compensation with different rates according to different stages of payment between the minimum payment line and top line.The average compensation rate of TB inpatients in county-level hospital was 47.1%.The rate for outpatient was 25.1%.Every TB inpatient and outpatient could get 1893 Yuan and 163 Yuan respectively as reimbursement from the NCMS in 2008.Conclusion The NCMS in Zhejiang province has relieved some financial burdens of TB patients.However,the reimbursement system needs to be improved,including raising the compensation rate particularly the rate for TB outpatients.We should consider NCMS as an important fund-raising method for TB prevention and control in the next decade.
