彩色多普勒超声诊断睾丸扭转及存活力的临床价值

Color Doppler ultrasound diagnosis of testicular torsion and survival prediction

目的:探讨彩色多普勒超声(color Doppler ultrasound,CDU)预测睾丸扭转复位术后存活力的临床价值。方法:回顾分析我院术后26例睾丸扭转患者CDU图像,根据不同超声表现将声像图分为A、B、C、D四种类型。将术后随访的睾丸存活力分为Ⅰ～Ⅱ级,Ⅰ～Ⅱ级睾丸存活,Ⅲ级睾丸萎缩。结果:26例中睾丸复位19例,切除7例。复位组睾丸存活10例,术前CDU图像为A、B、C型;萎缩9例,术前CDU图像分级C型7例,D型2例;术前CDU图像为D型9例,2例复位术后萎缩,7例术中切除。结论:阴囊CDU检查能够预测复位术后睾丸的存活力,为临床治疗提供重要参考依据。

Objective：To investigate the clinical value of color Doppler ultrasound（CDU） for predicting the testicular viability after reduction of testicular torsion.Methods：The CDU images in 26 cases of testicular torsion were retrospectively analyzed and divided into four types A-D according to the different CDU performances.The postoperative testicular viabilities during postoperative follow-up were divided into classification of Ⅰ-Ⅲ,class Ⅲ indicating testicular survival and class Ⅲ indicating testicular atrophy.Results：Among 26 cases of testicular torsion,reduction was in 19 cases and resection in 7 cases.There were 10 cases of testicular survival with preoperative A,B,C type CDU images in the reduction group and 9 cases of atrophy with preoperative CDU images of C type in 7 cases and D type in 2 cases.Preoperative CDU images were 9 cases of type D,2 cases of atrophy after reduction and 7 cases of intraoperative resection.Conclusion：Scrotum CDU examination can predict the viability of testicle after reduction,which provides important reference basis for clinical treatment.
