采用右房双连线射频消融治疗Ⅰ型心房扑动

摘要 采用解剖标志法沿三尖瓣环 (TA)至冠状静脉窦 (CS)和 CS至下腔静脉 (IVC)双连线射频消融治疗 5例 型心房扑动 (简称房扑 )。并在房扑终止后对双连线重复线性消融。 5例均成功终止房扑 ,平均放电 11.2± 1.9次、X线曝光时间 2 1.2± 5 .3 min、平均消融能量 3 2 4 0 0± 880 0 J、平均每靶点消融时间 1.88min,无手术相关并发症发生。随访 6~ 2 4个月 ,无一例复发。结论 :采用 TA至 CS和 CS至 IVC双连线射频消融可以成功阻断房扑 ;与其他方法相比复发率低 ,除放电次数略有增加外 ,射频消融时间无明显改变。 By use of anatomy sign method which depends on the double lineations from tricuspid annulus (TA) to coronary sinus (CS) and CS to inferior vena cava (IVC),type Ⅰ atrial flutter (AFL) was ablated.The linear ablation of the double lineations was repeated oncemore after AFL was terminated.Results:AFL in 5 cases were all terminated successfully,mean ablation frequencies:11.2±1.9 times,mean X ray fluoroscope time:21.2±5.3 min,mean ablation energy:32 400±8 800J,mean ablation time per target:1.88 min,and without complications.During the follow up of 6-24 months,none of them experienced AFL recurrence.Conclusion:AFL can be terminated successfully by way of ablating the double lineations (from TA to CS and CS to IVC).Comparing with other ablation methods,there is a low recurrence rate,a few increase of ablation frequencies,and no significant variation of ablation time.
