我院2009年万古霉素使用合理性分析

Analysis on Rationality of Vancomycin Used in Our Hospital in 2009

目的了解医院临床万古霉素的使用情况,分析其用药的合理性。方法参照卫生部颁布的《抗菌药物临床应用指导原则》,对2009年住院患者使用万古霉素的78份病历进行回顾性调查分析。结果 78例患者中76例进行了病原学检查,联合使用了抗菌药物的有41例,万古霉素的临床治疗有效率为61.54%;合理用药或基本合理用药50例(64.10%),不合理用药28例(35.90%)。结论万古霉素的使用基本合理,但应注意用药疗程,监测血药浓度,实行个体化给药方案,避免引起肾损害。

Objective To understand the application status of vancomycin in our hospital and to analyze its application rationality.Methods Referring to Antibacterial Drugs for Clinical Use Guiding Principle promulgated by the Ministry of Public Health,the medical records of 78 inpatients with vancomycin use in 2009 were retrospectively investigated and analyzed.Results Among 78 cases,76 cases had the etiological detection and 41 cases had combination use of antibiotics.The clinical effective rate of vancomycin was 61.54%.Rational drug use or basic rational drug use was in 50 cases（64.10%） and irrational drug use was in 28 cases（35.90%）.Conclusion The application of vancomycin in our hospital is basically rational.But attention should be paid to its medication course and blood concentrations monitoring,implement personalized medication scheme and avoid renal damage.
