Correlation of N-Terminal-pro-B-type natriuretic peptide with heart function and prognosis of heart failure patients

摘要 目的探讨N端脑钠肽前体(NT-proBNP)与心力衰竭患者心功能NYHA分级及左室射血分数(LVEF)的关系,研究NT-proBNP对心力衰竭患者的诊断及预后的临床价值。方法选取106例心力衰竭患者,心功能情况按照纽约心脏病协会(NYHA)分级,采用电化学发光法测定NT-proBNP的含量,心脏彩色多普勒超声测定左室结构和左室射血分数,研究NT-proBNP的变化与心功能NYHA分级及左室射血分数和预后的关系。结果 NT-proBNP水平与LVEF值之间呈负相关(r=-0.736,P<0.05),随着NYHA心功能分级的增加,患者的NT-proBNP含量升高,各组间比较具有显著性差异(P<0.05),动态监测NT-proBNP发现,治疗"有效"组的心衰患者NT-proBNP水平与治疗前比降低,而心衰"死亡"组患者在治疗后的NT-proBNP水平较治疗前基线下降少,甚至升高。结论 NT-proBNP能够反映心功能的状态,它对心力衰竭患者的诊断、治疗监测及预后的评估具有重要意义。 Aim To investigate correlation between N-Terminal-pro-B-type natriuretic peptide (NT-proBNP) and the the heart function of heart failure patients. Methods The levels of serum or plasma NT-proBNP in i06 heart failure patients classified by NYHA were measured by electrochemiluminescent immunoassay,the left ventricular structure and LVEF were examined by echocardio- graphy,last correlation was analysed with nonparametric test. Results The level of NT-pruBNP showed negative correlation with the value of LVEF (r=-0.736,P〈0.05). along with the increase of the grade of NYHA,the content of NT-proBNP went up. the level of NT-proBNP in patients with valid treatment show distinct decrease through dynamicly monitoring, but the level of NT-proBNP in patients died of heart failure were higher than before treatment. Conclutions The level of NT-pmBNP can reflect the state of the heart function and it is of important significance in diagnosis, treatment, monitorit and prognosis of heart failure failure.
