摘要
目的探讨休克期切痂对严重烧伤后凝血和纤溶功能变化的影响。方法采用犬40%TBSAⅢ度烫伤模型,分为休克期切痂组(E组,于伤后3 h切痂)和非切痂组(C组),分别检测血浆中TM、D-二聚体的含量。结果伤后3 h两组TM、D-二聚体均显著高于伤前(P<0.05),24 h后E组TM水平显著低于C组(P<0.05),D-二聚体也明显低于C组(P<0.05)。结论休克期切痂可减轻内皮细胞损伤,预防或减少血栓的形成。
Objective To investigate the influence of escharectomy during burn shock stage upon the functional changes of fibrinolysis and coagulation following severe burns. Methods A model of 40% TBSA m degree burn was used in dogs,which was divided into 2 groups: escharectomy during burn shock stage group( group E, undergoing escharectomy at 3h postburn) and unescharectomy group ( group C). TM and D - dimer were measured. Results The levels of TM and D - dimer were higher at 3h after injury than before injury in both groups (P 〈 0. 05 ), The level of D - dimer was lower significantly in group E than in group C after 24h postburn ( P 〈 0.05 ), the level of TM was too ( P 〈 0. 05 ). Conclusion Escharectnmy during burn shock stage was beneficial in reducing injury of endothelial cells,prevent or decrease the micro - thrombosis.
