应用人工股骨头置换术治疗老年股骨粗隆间骨折的观察

The study of femoral head prosthesis replacement of aged patients treated intertrochanteric fracture

目的探讨人工股骨头置换术对老年股骨粗隆间骨折进行治疗的临床相关问题。方法将本院2007～2010年收治的老年股骨粗隆间骨折患者43例随机分为两组。人工股骨头置换术治疗患者24例为治疗组,动力髋螺钉治疗内固定治疗患者19例为对照组。将两组患者下床时间、手术时间、住院时间、出血量及并发症等指标进行比较。结果治疗组术后4～7 d可以下床,手术时间(60±8)min,住院时间(17±2)d,平均手术出血量428 ml,尿路感染2例,坠积性肺炎1例,精神障碍1例。住院时间、下床时间和并发症方面,两组比较,差异有统计学意义(P<0.05)。结论人工股骨头置换术具有离床早、住院时间短等优势,可提早使老年患者术后恢复正常的生活。

Objective To investigate the features of the method of femoral head prosthetic replacement for advanced age patients with intertrochanterle fracture. Me/hods 43 advanced age patients with intertrochanterie fracture were involved, and sorted into two groups. 24 patients in treatment group were treated with femoral head prosthetic replacement; 19 patients in control group were treated with dynamic hip screw. Related Indicator were compared between the groups. Results Patients in treatmend group can get up of bed for action 4 - 7 d after surgery ; the duration of surgery was 60 ± 8 min ; length of slay was 17 ±2 d; quantity of bleeding was 428 ml; and one patient got urinary tract infection,one got hypostatic pneumonia,one got mental disorder. The duration of surgery, the days to recover to get up of bed for action, and complications were significant dif- ferent between the groups（P 〈 0. 05 ）. Conclusion The method of femoral head prosthetic replacement has superiorities of less days to recover and shorter length of stay.
