PK刀与缝合联合在腹腔镜卵巢良性囊肿剔除术中的应用

The application of scaple PK combined to suturation in laparoscopy rejecting to ovaries cysto-optimum

目的探讨腹腔镜卵巢良性囊肿剔除术中联合使用PK刀及缝合应用价值及临床效果。方法单侧卵巢囊肿拟行腹腔镜囊肿剔除术患者68例,随机分为两组,试验组合对照组,试验组:术中联合PK刀和缝合进行创面止血;对照组:仅使缝合止血。所有患者术中统计出血量、手术时间,并分别于术前、术后3个月的月经周期的第3天取静脉血测促卵泡成熟素(FSH)、促黄体生成素(LH)和雌二醇(E2),并行术后随访。结果相对于对照组,试验组手术时间、术中出血量明显减少,两者有显著性差异(P<0.05);术后3个月,两组均出现E2下降及FSH、LH增高,与术前相比,无显著性差异(P>0.05);两组间FSH、LH、E2比较,均无显著性差异(P>0.05)。两组月经改变也无显著性差异(P>0.05)。结论腹腔镜卵巢囊肿剥除术联合使用PK刀及缝合止血和单纯使用缝合止血比较手术时间短、术中出血少、对卵巢近期功能无明显影响,且更易掌握和使用,值得推广。

Objective To discuss the values and clinical effects of Scaple PK combined to suturation in laparoscopy rejecting to ovaries cysto-optimum.Methods The 68 patients which plan to remove the half of the ovaries cysto-optimin laparoscopy were divided into experimental and control groups randomly,the experimentatal group：Scaple PK combined to suturation to stop bleeding of the wound surface,the control：suturation stop bleeding merely.All the patients calculate the volume of blood loss,operation time and detect follicle-stimulating hormone（FSH）,luteotropic hormone（LH）and estradiol（E2）in venous blood on the 3rd day in menstrual cycle after 3 months of operation,together with follow-up visit after operation.Results Compare to the control,the experimental group time-consuming and bleeding-volume decrease obviously,which had significant difference（P0.05）,all of the two groups appear descent of E2 and up-rengulation of FSH and LH after 3 months of operation,which have not statistical significance contrast to the Preoperative（P0.05）,and the two groups menstruous change without significant difference,either（P0.05）.Conclusion Contrast to the suturation hemostasis merely,the technique which laparoscopy rejecting to ovaries cysto-optimum of Scaple PK combined to suturation have the dominants of time-voulme shorter,haemorrhagia less,and no obviously effect on the ovaries fuction in the near future,furthermore,which is easier to mastered and emploied,deserved to generalization.
