摘要
目的:用中医宏观和西医微观方法观察IgA肾病的临床表现和病理特点,并探讨其发病机制和治疗方法。方法:病例选自我院1994年7月至2009年10月诊断明确的IgA肾病562例,根据患者年龄、性别、病程、临床表现,病理类型和分级,随机分为中西医结合治疗组432例,西医对照组130例,分别观察半年和1年后统计疗效。结果:治疗半年后治疗组总有效率73.15%,对照组总有效率44.62%,两组比较P<0.01;治疗1年后治疗组总有效率91.67%,对照组总有效率69.23%,两组比较P<0.01。结论:宏观与微观相结合辨证治疗IgA肾病可以明显提高疗效,减轻激素和环磷酰胺的副作用,减少复发率。
Objective：To investigate the common viewpoint on clinical and pathologic characteristics and pathogenesis of IgA nephropathy（IgAN）by the macroscopical means of traditional Chinese Medicine（TCM）and the microscopical means of modern medicine（MM）;also to investigate the treatment methods of combined TCM and MM on IgAN.Methods：Diagnosed by renal needle biopsy,562 cases with IgAN from April 1994 to April 2010 were randomly divided into the western medicine group（WM group,n=130）and the group treated by combined way of TCM and MM（TM group,n=432）,according to age,sex,course of disease,clinical manifestation,pathologic type and ranking.Therapeutic effects were analyzed by way of statistics after six months＇and one year＇s treatment respectively.Results：After sex months＇and one year＇s treatment,the general effective power of WM group is 44.62% and69.23% respectively,while that of TM group is 73.15% and 91.67% respectively compared with WM group（P0.01）.Conclusion：The combination of TCM and MM can boost therapeutic effect and abate the side effect of glucocorticoid and cyclophosphamide,and can reduce recurrence rate definitely in IgAN treatment.
