布鲁杆菌病76例疗效分析

Analysis of therapeutic effects for 76 cases of brucellosis

目的探讨急性和亚急性期布鲁杆菌病不同方法治疗的临床效果。方法将76例急性和亚急性期布鲁杆菌病患者按数字表法随机分为观察组（40例）和对照组（36例）。观察组给予利福喷汀、多西环素和头孢他啶；对照组给予利福平、多西环素，疗程均为6周，随访6个月。结果观察组发热、多汗、关节痛、肝脾及淋巴结肿大等消失时间较短，与对照组相比差异有统计学意义（t=7．37、4．62、4．18、3．86，P〈0．01）。观察组血常规、血沉恢复及血或骨髓培养转阴时间比对照组明显缩短（t=6．39、5．42、5．78，P〈0．01）。临床疗效，观察组中，治愈36例，有效2例，总有效率95．00％；对照组中，治愈24例，有效4例，总有效率77．78％。两组患者总有效率比较，差异有统计学意义（X^2=4．92，P〈O．05）。结论急性和亚急性布鲁杆菌病病原治疗，联合应用利福喷汀、多西环素和头孢他啶疗效好，值得应用。

Objective To explore the curative effects for acute and subacute brucellosis by different therapeutic methods. Methods 76 cases of acute and subacute of brucellosis patients were randomly divided into two groups （40 eases in survey group and 36 eases in control group）. The patients in control group were given rifampin, doxyeyeline, while the survey group were given rifapentine, doxyeycline and ceftazidime. All the patients were treated for 6 weeks and foUowed-up for 6 months. Results The symptoms of fever, hyperhidrosis, arthralgia, enlargement of lymph nodes in liver and spleen in suvey group disappeared in a short time as compared with control group, there were statistical differences（ t = 7.37,4.62,4.18,3.86, P 〈 0.01）. The normalized time of routine blood, ESR and blood or bone marrow culture in survey group were significantly shorter than those in control group （ t = 6.39, 5.42, 5.78, P 〈 0.01）. The total effective rate was 95.00% in survey group （36 eases were curative, 2 cases had therapeutic effects）; wilde total effective rate in control group was 77.78% （24 cases were curative, 4 cases had therapeutic effects）. There was a statistical significance between the two groups （ X^2 = 4.92, P 〈 0.05）. Conclusions Clinical effect of rifapentine, doxycycline ceftazidime combination for acute and subacute brucellosis is good, it is worth for application.
