左心房容积追踪技术评价高血压患者左心房功能 被引量:5

摘要 目的应用左心房容积追踪技术(LA Tracking)探讨不同左室壁厚度高血压患者左房功能的差异。方法根据JNC7高血压诊断标准及左室质量指数,将患者分为高血压非左室肥厚组(NLVH组)28例、左室肥厚组(LVH组)30例,对照组32例。在LA Tracking模式下获取左房容积时间曲线,记录左房最大容积(LAVmax)、心电图P波对应左房容积(LAVp)、左房最小容积(LAVmin),用体表面积(BSA)标化(LAVImax、LAVIp、LAVImin),再计算反映贮存器功能的指标左房整体排出量(LAVItotal)、左房扩大率(%LAVItotal);反映管道功能的指标被动左房排出量(LAVIpass)、左房被动排出率(%LAVIpass)及有效左房被动排出率(%eLAVIpass);反映助力泵功能的指标主动左房排出量(LAVIact)、左房主动排出率(%LAVIact)及有效左房主动排出率(%eLAVIact)。结果与对照组比较,NLVH组及LVH组LAVItotal、LAVIact、%LAVIact、%eLAVIact增加,LAVIpass、%LAVlpass、%eLAVI-pass降低。与LVH组比较,NLVH组LAVItotal、LAVIact增加,LAVIpass、%LAVlpass降低。结论高血压患者左房贮存器、管道及助力泵功能在左室肥厚前已发生变化,且随着左室肥厚、左室舒张功能受损的增加,变化更为显著。LA Traching技术能敏感、准确地评价左房功能。
