重度阻塞性睡眠呼吸暂停征的持续正压通气治疗分析 被引量:5

摘要 [目的]探讨分析持续正压通气治疗对重度阻塞性睡眠呼吸暂停征(OSAHS)的疗效。[方法]选择经多导睡眠监测检查为重度OSAHS患者且行智能型经鼻正压通气(AUTO SET)治疗3个月的45例患者为对象。对呼吸暂停低通气指数(apneahyponeaindex,AHI)、最低/平均血氧饱和度(LSaO2/MSaO2)、平均血压等指标进行观察。[结果]治疗后患者打鼾、呼吸暂停和白天嗜睡等症状明显改善。AHI:(10.36±2.27)次/h,LSaO2:(85.91±5.37)%;MSaO2:(92.87±5.56)%,平均血压24 h SBP:(135.69±8.31)mmHg,24 h DBP:(87.42±6.65)mmHg。与治疗前AHI:(61.23±5.38)次/h,LSaO2:(42.45±5.78)%,MSaO2:(75.12±5.68)%,24 h SBP:(165.21±7.97)mmHg,24 h DBP(105.62±5.43)mmHg相比均有明显变化,差异具有显著性意义(P<0.05)。[结论]重度阻塞性睡眠呼吸暂停征经过AUTO SET治疗可缓解患者临床症状及改善睡眠指标。 [Objective] To investigate the effects of continuous positive airway pressure(CPAP) on severe OSAHS.[Methods] Forty-five severe OSAHS cases checked by polysomnography and treated by intelligent nasal continuous positive airway pressure(AUTO SET) for 1 year were selected for this study.All patients′ apnea hyponea index(AHI),lowest and average oxygen saturation(LSaO2/MSaO2),and blood pressure were measured.[Results] After treatment,the apnea index AHI:(10.36±2.27)/h,LSaO2:(85.91±5.37)%,MSaO2:(92.87±5.56)%,mean blood pressure 24 h SBP:(135.69±8.31)mmHg;24 h DBP:(87.42±6.65)mmHg showed significantly change(P0.05) if compared to pre-treatment data AHI:(61.23±5.38)/h;LSaO2:(42.45±5.78)%,MSaO2:(75.12±5.68)%,24 h SBP:(165.21±7.97)mmHg,24 h DBP:(105.62±5.43)mmHg.[Conclusion] Severe symptoms of obstructive sleep apnea can be improved if it is treated by AUTO SET.
