成人单发性肥大细胞瘤致皮肤松弛症

Cutis laxa acquisita associated with adult form solitary cutaneous mastocytoma

报告1例成人单发性肥大细胞瘤致皮肤松弛症。患者女,33岁。左侧腋前淡红色丘疹5年,近1年局部丘疹渐消退遗留萎缩斑。皮肤科检查:左腋前一直径2 cm的萎缩斑,表面皮肤松弛,以色素减退为主,局部可见淡红斑,Darier征阳性。皮损(淡红斑处)组织病理检查:表皮轻度角化过度,毛囊角栓,棘层轻度不规则增厚,真皮浅中层可见淋巴细胞、组织细胞和多量的疑为肥大细胞的立方形细胞浸润,弹性纤维染色可见弹性纤维断裂,局部缺如;甲苯胺蓝染色可见较多呈蓝紫色结晶的肥大细胞。诊断为成人单发性肥大细胞瘤致皮肤松弛症。

A case of cutis laxa acquisita associated with adult form solitary cutaneous mastocytoma is reported. A 33 year-old female complained of a lesion on her left axiUa for 4-5 years. She found occasionally a hard red papule appeared on her left axilla 5 years ago. She felt itching, especially after scratch. The involved skin became slack during recent one year, but the papule disappeared gradually. Physical examination revealed a slack pink patch sized in 2cm in diameter on her left axilla, with hypopigmention. Darier＇s sign was positive. Biopsy and histopathology showed the gently hyperkeratosis and acanthosis. There were the infiltrations of lymphocytes, histiocytes and lots of suspensive mast cells in the upper and middle dermis. The broken elastic fibers were observed by elastic tissue staining. TB （toluidine blue） showed ianthinus crystal in the mast cells. A diagnosis of curls laxa aquisita associated with adult form solitary cutaneous mastocytoma was made based on the clinical and histopathological manifestations.
