乳腺恶性肿瘤127例临床病理分析

Clinicopathological characteristics of breast malignant tumors:an analysis of 127 cases

目的探讨乳腺恶性肿瘤的临床病理特征。方法回顾性分析127例乳腺恶性肿瘤的临床特点、病理特征及免疫组化表现。结果乳腺恶性肿瘤占乳腺肿瘤的10.36%,良恶性比例为8.65:1,发病率在10年间由4.72%上升至11.81%,呈逐年升高趋势;发病高峰年龄为40～49岁(36.22%);病灶均为单侧、单个包块,左侧多于右侧,左右两侧比为1.15:1。肿瘤直径以2～5cm最多见(54.33%)。病理类型以浸润性导管癌(非特殊性)为最多见(81.10%)。可评估的96例中有65例(67.71%)有不同程度的淋巴结转移。免疫组化ER、PR和c-erbB-2阳性表达率分别为56.00%、53.33%和56.00%;Ki67阳性表达率100.0%增殖指数平均为21%;p53阳性表达率为55.00%。结论乳腺恶性肿瘤是危害女性生命健康的主要肿瘤,预后与患者年龄、病理分型、腋窝淋巴结转移情况及免疫组化等有密切关系,早诊断和早治疗可以提高生存率。

Objective To investigate the clinicopathological characteristics of breast malignant tumors.Methods The clinical and pathological features and immunohistochemical manifestations were retrospectively analyzed in 127 patients with breast malignant tumor.Results Breast malignant tumors accounted for 10.36% of breast tumors.The ratio of benign and malignant tumors was 8.65：1.The incidence of breast malignant tumor increased gradually from 4.72% to 11.81% in the recent decade.The pathogenic peak age was 40-49 years old（36.22%）.All foci were unilateral and single mass.The incidence of left breast cancer was more common than that of right breast,and the ratio was 1.15：1.The usual diametral size of neoplasm was from 2 to 5 cm（54.33%）.The invasive ductal carcinoma was the most common pathological type（81.10%）.The lymphonodus metastasis was found in 65 cases（67.7%） of assessable 96 patients.The expression of assessable estrogen receptor（ER）,progtrogen receptor（PR） and c-erbB-2 were 56.00%,53.33% and 56.00%,respectively.The positive expression rate of ki67 was 100.0%,and the proliferation index of Ki67 was 21% averagely.The positive expression rate of p53 was 55.00%.Conclusions The malignant breast tumor is the most common neoplasm endangering health and life of women.The prognosis is closed correlated with the age,pathological type,metastasis of the armpit lymphonodus and the immunohistochemical features.Earlier diagnosis and earlier treatment would improve the survival rate of malignant breast tumor.
