无需血管及消化道重建切除侵犯腹腔干的胰体癌：改良Appleby手术

摘要
胰体癌由于临床症状出现晚，确诊时常常为局部进展期，难以实施根治性切除手术。侵犯邻近重要血管是病变不可切除的重要原因之一。NCCN指南v．2．2010指出，胰体癌不可切除的标准之一是腹腔干被肿瘤侵犯包绕超过180°。但是，如果能够连同受侵犯的血管整块切除肿瘤，不仅可以提高手术切除率，改善生活质量，而且部分患者可能延长生存期。
