腔镜下甲状腺切除术的临床应用(附53例报告)

Application of endoscopic thyroidectomy in clinical practice with a report of 53 cases

目的探讨腔镜下行甲状腺切除术的可行性及临床应用价值。方法 2003年10月-2010年1月对我院53例甲状腺不同疾病患者(26例甲状腺腺瘤、14例结节性甲状腺肿、2例甲状腺功能亢进和11例甲状腺癌)行腔镜下甲状腺手术。结果完成手术52例,其中单孔腔镜手术3例,中转1例。平均手术时间90(62-210)min,术中出血5-50ml,术后平均住院5d。2例出现暂时性喉返神经麻痹,无甲状旁腺损伤等其他并发症发生。术后随访45例,时间8个月-5年,平均24月,患者均对手术治疗效果满意。结论腔镜下甲状腺切除术安全可行,颈部无疤痕,具有很好手术及美容效果。

Objective To study the possibility and clinical value of laparoscopic thyroidectomy. Methods Fifty-three patients （26 with thyroid adenoma, 14 with nodular goiter, 11 with thyroid carcinoma and 2 with hyperthyroidism） underwent endoscopic thyroidectomy in our hospital from October 2003 to January 2010. Results The endoscopic thyroidectomy was successful in 52 patients. Of these patients, I received single-port laparoscopie operation, 1 was transferred to open surgery. The mean operation time was 90（62-210）mln, the blood Joss was 5-50mi, and the mean hospital stay time was 5 days. Forty-five patients were followed up for 8 months to 5 years （mean 24 months） after operation. All the patients were satisfied with the outcome. Conclusion Laparoscopie thyroidectomy is a safe procedure with a good cosmetic effect and no scar formation.
