大疱性类天疱疮患者临床特征和系统损害与性别的相关性

The Correlation among Gender,Clinical Characteristics and Systemic Damage of Bullous Pemphigoid

目的评价BP系统损害与性别的相关性。方法回顾性分析113例BP患者的临床表现、实验室指标和治疗方法。结果 BP患者临床表现与性别不相关(P>0.05);但女性患者血红蛋白量降低的发生率和尿液中白细胞数明显高于男性,男性患者的血尿水平及治疗前后血ALT,BUN和GGT升高的水平均明显高于女性患者(P均<0.05)。另外,有肝肾损害者治疗时激素起始用量较无肝肾损害者高,住院时间也较后者长,两者差异均有统计学意义(P均<0.001)。结论 BP患者肝肾损害在性别上存在差异,有或无肝肾损害影响患者的治疗和预后,应早期针对肝肾损害情况制定治疗方案,以减少疾病复发。

Objective To compare the clinical characteristics of bullous pemphigoid （BP） in male and female patients correlation between gender and systemic damage of BP. Methods A retrospective analysis was performed on clinical figures, laboratory findings, treatment and the prognosis of the 113 BP patients. Results There was no significant difference between gender in clinical manifestations of BP（ P 〉 0.05 ）. In female patients The incidence of lower hemoglobin and urine WBC count in were significantly higher than in male. The level of hematuria and the elevated level of blood ALT, BUN, and GGT before and after treatment in male were significantly higher than that in female（P 〈 0.05 both）. In addition, whether have liver and kidney damage or not, the starting dose of corticosteroid therapy and the time in hospital have significant difference （ P 〈 0.001 both）. Conclusion The differences between gender in liver and kidney damage of BP, which im- pacted the treatment and prognosis is distinct and should be pay attention to, especially in male. The further formulate treatment program should be taken to reduce the recurrence of the disease.
