肺炎支原体肺炎患儿外周血白细胞和炎性因子检测的临床意义 被引量:5

Clinical significance of detection leucocyte in peripheral blood and inflammatory factors of mycoplasma pneumoniae pneumonia

摘要 目的探讨外周血白细胞计数及分类、C反应蛋白(CRP)、P物质(SP)、降钙素原(PCT)和基质金属蛋白酶-9(MMP-9)的变化对肺炎支原体肺炎(MPP)的诊断价值。方法选择35例MPP急性期患儿作为感染组,35例健康体检儿童作为正常对照组。2组均采用Sysm ex XE2100五分类全自动血细胞分析仪检测白细胞及其分类;采用日立7170全自动生化分析仪检测外周血CRP含量;利用放免方法检测SP浓度;PCT水平的变化采用免疫发光方法进行测定;MMP-9的检测采用ELISA方法。结果 MPP急性期患儿与对照组比较白细胞总数略有升高,但无统计学意义(P>0.05);白细胞分类中性粒细胞百分数增高(P<0.05)、淋巴细胞百分数降低(P<0.05)、单核细胞百分数增高(P<0.01);血清CRP、SP、PCT和MMP-9含量与对照组比较显著增高(P<0.001)。结论 MPP急性期患儿外周血中白细胞分类和炎性因子变化规律对疾病的诊治具有重要的指导意义。 Objective To explore the diagnostic value of peripheral blood leukocytes count and classification,C-reactive protein(CRP),substance(PSP),procalcitonin(PCT) and matrix metalloproteinases-9(MMP-9) in children mycoplasma pneumoniae pneumonia(MPP).Method Selecting 35 MPP children in acute stage as infection group and 35 healthy children as control group.Peripheral blood leukocyte count and classification were determined by Sysmex XE2100 auotomatic blood cell analyzer,the level of CRP was measured by 7170 automatic analyer,the concentration of SP was detected by RIA,the concentration of PCT was measured by luminescent immunoassay,and level of MMP-9 was determined by ELISA.Results The peripheral blood leukocyte count of infection group was higher than control group,but it was no significant difference between two groups(P0.05),the proportion of neutrophil was higher and the proportion of lymphocyte was lower than control group(P0.05),while the proportion of monocyte in MPP group was much higher than healthy children(P0.01).The levels of CRP,SP,PCT and MMP-9 in MPP children in actue stage were remarkably higher than that in control group(P0.001).Conclusions Peripheral blood leukocyte classification and inflammatory factors had important directive in diagnosis and treatment of children MPP.
