摘要
目的探讨舒芬太尼后处理对在体大鼠心肌缺血再灌注损伤的影响。方法健康雄性SD大鼠经结扎冠状动脉左前降支制备心肌缺血再灌注损伤模型后随机分为8组:假手术(A)组:只穿线,不结扎;缺血再灌注(B)组:缺血30min,再灌注120 min,再灌注前5 min单次静脉注射生理盐水1 ml;缺血后处理(C)组:缺血30 min末行缺血10 s,再灌注10 s,重复3次后再灌注120 min;舒芬太尼后处理(D~H)组:再灌注前5 min分别单次静脉注射舒芬太尼0.1、0.3、1、3、10μg/kg,5 min后再灌注120 min。于结扎线缝好后平衡30 min(T0)、缺血30 min末(T1)、后处理末(T2)、再灌注120min末(T3)记录血流动力学参数。计算心肌梗死面积(IS)与缺血危险区(AAR)比值(IS/AAR)。选取A、B、C、最佳剂量舒芬太尼后处理组于T3时取颈动脉血2 ml,采用硫代巴比妥酸(TBA)法测定丙二醛(MDA)浓度,黄嘌呤氧化酶法测定超氧化物歧化酶(SOD)活性。结果与B组比较,C、E、F、G、H组IS/AAR降低(P<0.05),其中舒芬太尼后处理组中F组降低最为明显。舒芬太尼ED50为0.031 74μg/kg。与A组比较,其余3组血清MDA浓度升高,SOD活性降低(P<0.05);与B组比较,C、F组MDA降低,SOD活性升高(P<0.05)。结论舒芬太尼可模拟缺血后处理减轻在体大鼠心肌缺血再灌注损伤,并且呈剂量依赖性。
Objective To investigate the effects of sufentanil postconditioning(SpostC) on myocardial ischemia-reperfusion(I/R) injury in rats.Methods Forty-eight male SD rats were ligated.the left anterior descending(LAD) coronary artery toform a I/R injury model.Then animals were divided randomly into 8 groups:Ⅰ.Rats received no occlusion or reperfusion(group A);Ⅱ.Rats were subjected to 30 min ischemia and 120 min reperfusion,for a negative control group,normal saline 1 ml was infused at the 25 min of ischemia(group B);Ⅲ.Immediately at the onset of reperfusion,rats underwent 3 intermittent cycles of 10s reperfusion alternating with 10s ischemia(groupC);Ⅳ-Ⅷ.Rats received randomly an intravenous infusion of sufentanil diluted with normal saline 1 ml of either dosage(0.1,0.3,1,3,10 μg/kg) at the 25 min of ischemia(group D-group H).MAP,HR and RPP were recorded after a 30 min stabilization(T0),at the end of ischemia(T1),at the end of postconditioning(T2) and at the end of reperfusion(T3).The infact size(IS) and the area at risk(AAR) were measured by TTC staining.Serum MDA concentration and SOD activity were determined in group A,group B,group C and group optimal sufentanil postconditioning.Results Compared with group B,IS/AAR was significantly decreased in the treatment groups except from group D and group F had the optimal protective effect in the sufentanil postconditioning groups(P0.05).The sigmoidal ED50 was 0.0317 4 μg/kg.The serum MDA concentration was significantly increased in group B,C and F,and the serum SOD activity significantly decreased in the three groups as compared with group A(P0.05).Group C and F attenuated the I/R-induced increasing in serum MDA concentration and decreasing in SOD activity as compared with group B(P0.05).Conclusion Sufentanil postconditioning can protect myocardium against I/R injury and the effect is dosage-dependent.
