PCEA用于剖宫产术后镇痛的效果分析

目的探讨剖宫产术后芬太尼硬膜外自控镇痛(PCEA)的效果分析.方法选择2008年6月至12月于张家口第五医院及张家口妇幼保健院进行剖宫产的400例产妇为研究对象,将其随机分为观察组(PCEA组)200例,术毕硬膜外导管连接PCEA芬太尼混合液装置;对照组(杜非合剂镇痛组)200例,术后6h肌肉注射杜非合剂半量.观察镇痛效果和对产妇的影响.结果观察组术后不同时段的镇痛效果和哺乳情况明显优于对照组,差异有统计学意义(P<0.05),但产后出血、子宫复旧情况及排气时间长短差异无统计学意义(P>0.05).结论剖宫产后PCEA芬太尼镇痛效果确切,促进母乳喂养成功实施,未增加产后出血量,利于产后恢复.

Objective To evaluate the effect of patient-controlled epidural analgesia（PCEA）with fentanyl after Cesarean section.Methods 400 patients scheduled for Cesarean section in No.hospital and Maternal Child Care Service Centre of Zhang Jiakou from June to December of 2008 were enrolled in this study.The patients were randomly divided into PCEA group（200 cases）and control group（200 cases）.Patients in the PCEA group received PCEA with fentanyl after Cesarean section.while for those in the control group,intramascular injected dolantin and promethazine in half of the dose were administrated six hours after surgery.The analgesia effect and the influence to the patients were observed.Result The post-operative analgesia effect and the breast feeding rate were significantly better in PCEA group than that of control group,and the difference was statistically significant（P0.05）.However,there was no significant difference（P0.05） for the postpartum hemorrhage,but ther was no significant difference in recovery of uterus and bowel function between the two groups.Conclusion The result of the current study indicated that the post-operative PCEA with fentanyl after Cesarean section was effective,could promote the breast feeding rate,didn＇t increase the postpartum hemorrhage and therefore could help the postnatal recovery.
