硝普钠治疗急性左心衰竭32例疗效观察

Effect of nitroprusside sodium on acute left ventricular failure

目的:探讨硝普钠治疗急性左心衰竭的临床疗效。方法:选择我院2008年7月至2010年7月急性左心衰竭患者64例,将所选患者随机分为两组,观察组和对照组各32例。对照组患者采用常规吸氧、给予强心药物、利尿、镇静及静脉滴注硝酸甘油等,同时控制诱发因素。观察组在对照组治疗基础上给予硝普钠替换硝酸甘油。硝普钠50 mg加入5%葡萄糖注射液50 mL中经输液泵静脉滴注,起始滴速为5μg.kg-1.min-1,根据患者反应逐渐增加滴速,递增速度为0.5μg.kg-1.min-1,静脉滴注时要注意避光。观察两组患者用药期间临床症状和体征的改善情况;监测心电图、血氧饱和度等。结果:观察组总有效率93.75%,对照组总有效率75.00%,两组总有效率比较差异有统计学意义(P<0.05)。结论:硝普钠能够在短时间内改善急性左心衰竭患者临床症状和体征,临床效果显著,值得借鉴。

Objective： To explore the effect of nitroprusside sodium on acute left ventricular failure.Methods： A total of 64 cases with acute left ventricular failure admitted from July 2008 to July 2010 were selected,and randomly divided into observation group and control group by half.Patients in control group were treated by conventional oxygen inhalation,cardiotonic,diuresis drugs,sedative drugs and venous dripping of nitroglycerin etc and by controlling of causative factors.Instead of nitroglycerin,patients in observation group were treated with 50 mg nitroprusside away from light,adding into 50 mL 5% glucose solution.The beginning speed was 5 μg·kg-1·min-1 and was increased by 0.5 μg·kg-1·min-1.The improvement of symptoms and physical signs were observed,and ECG and saturation of blood oxygen were monitored.Results： Total efficacy in observation group was 93.75%,and 75.00% in control group with significant difference between two groups（P〈0.05）.Conclusions： Nitroprusside can improve symptoms and physical signs in short time.It has significant effect on acute left ventricular failure.
