内固定加外固定架在不稳定型骨盆骨折治疗中的应用

Application of internal fixation and external fixator for unstable pelvic fracture

目的探讨内固定加外固定支架手术在不稳定型骨盆骨折治疗中的，临床应用及疗效。方法对我院自2001年3月至2009年6月收治的137例不稳定型骨盆骨折患者的临床资料进行回顾性分析，本组患者均采用肉固定加外固定支架手术治疗。其中车祸交通伤81例，高处坠落伤34例，塌方挤压伤22例。根据Tile’s分型，本组中B型89例，C型48例。所有患者术后均获随访，随访时间1～6年。结果本组中125例术后关节功能明显改善，疼痛缓解，各种并发症发生率低。采用Matta评定标准进行评估，术后优良率达91．3％。结论内固定加外固定支架手术治疗不稳定型骨盆骨折的临床疗效良好，能够重建骨盆稳定性，是治疗此类骨折的有效方法。

Objective To evaluate the clinical application and effect of internal fixation and external fixator for treatment of unstable pelvic fracture. Methods From March 2001 to June 2009,137 pa- tients with unstable pelvic fractures were treated by internal fixation and external fixator, a retrospective study was conducted. The injuries were caused by the traffic accident in 81 patients, following falling from high places in 34 patients, crush injury by collapse in 22 patients. According to Tile classification, 89 cases were Tile B ,48 cases were Tile C. All the patients were followed up for 1 to 6 years. Results Among the 137 patients ,excellent results were achieved in 125 cases with the improvement in joint function and easement of pain,with lower complications. All the patients were reviewed using the Matta evaluation system, the result showed that the clinical effective rate exceeded 91.3%. Conclusions Internal fixation combined with external fixator is a very good method, which was effective for the treatment of unstable pelvic fracture, it may be the most effective way to rebuild the stability of pelvic.
