Distribution of Pathogenic Bacteria of Hospital Infection in ICU and Drug Resistance Analysis in Our Hospital during 2008-2010

摘要 目的:了解ICU院内感染病例菌株分布及常用抗菌药物的耐药情况。方法:对我院2008-2010年ICU住院患者各类送检标本细菌培养、分离鉴定和药敏试验结果进行回顾性分析。结果:院内感染病例中共分离出病原菌856株,其中革兰阴性杆菌617株,占72.1%;革兰阳性球菌164株,占19.2%;真菌75株,占8.8%。革兰阴性杆菌对第3、4代头孢菌素耐药率较高,对亚胺培南、哌拉西林/舒巴坦、阿米卡星敏感性较好,亚胺培南总耐药率最低。革兰阳性球菌对万古霉素的耐药率为0%。结论:分离的病原菌以革兰阴性杆菌为主,所占比例有逐年升高趋势,对抗菌药物的耐药率总体上有上升趋势,且耐药性不断发生变化。 OBJECTIVE:To investigate the distribution of pathogenic bacteria of hospital infection in ICU and drug resistance of common antibiotics. METHODS: All the separation identification and drug sensitivity test of pathogens isolated from inpatients with hospital infection from 2008 to 2010 were analyzed retrospectively. RESULTS: 856 pathogenic bacteria were isolated in hospital infection cases of ICU. The main pathogen of nosocomial infection was 617 strains of gram-negative bacteria (72.1%), 164 strains of gram-positive bacteria (19.2%) and 75 strains of fungi (8.8%) . Drug resistant rate of gram-negative bacteria to 3/4-generation cephalosporins was high, while gram-negative bacteria was sensitive to imipenem, piperacillin/sulbactam sodium and amikacin. Drug resistance rate of gram-negative bacteria to imipenem was the lowest. Drug resistant rate of gram-positive bacteria to vancomycin was 0%. CONCLUSION: Isolated pathogenic bacteria are mainly gram-negative bacteria. The proportion of gram-negative bacteria is rising year by year, and drug resistance of gram-negative bacteria to antibiotics is rising on the whole. Drug resistance of bacteria is constantly changing.
