经颅多普勒与脑干听觉诱发电位在煤工尘肺低氧血症中的联合应用观察

摘要 目的:分析煤工尘肺合并低氧血症患者脑干听觉诱发电位(BAEP)和经颅多普勒(TCD)的改变及特点,研究煤工尘肺患者长期慢性脑部缺氧对脑干和脑血管功能的影响。方法:选择60例煤工尘肺低氧血症患者和60例自愿受试者分别进行BAEP和TCD测试,测定Ⅰ、Ⅲ、Ⅴ各波潜伏期(PL)、峰间潜伏期(IPL)、两耳间PL或/IPL之差(ILD),峰间期Ⅰ-Ⅲ/Ⅲ-Ⅴ之比,间侧Ⅴ/Ⅰ波幅比等数值、MCA、ACA、PCA、VA、BA等血管的血流速度、脉动指数等。结果:低氧血症组BAEP异常率和TCD所测血流速度同对照有显著差异,TCD表现为多支血管血流速度降低。结论:脑干听觉诱发电位和经颅多普勒检查是检查煤工尘肺合并低氧血症患者脑部功能障碍的敏感指标,两项联合应用更好的观察对煤工尘肺合并低氧血症者的脑部功能改变。 Objective:To analyze the changes and characteristics of BAEP and TCD of Coal Mine workers'pneumoconiosis with hypoxemia so that study the influence of chronic anoxic in the function of brainstem and blood vessel of brain.Methods:BAEP and TCD were detected in 60 CMWP(group A)and 60 volunteers(Group B)respectively.The PL and IPL of wave I,Ⅲ,Ⅴ,ILD,Ⅰ-Ⅲ/Ⅳ-Ⅴ and Ⅴ/Ⅰwere detected.Blood flow velocity and pulsatile index of MCA,ACA,PCA,VA,BA were detected.Results:The abnormal rate of BAEP and blood flow velocity of group A are obviously different with the group B,while TCD showed the blood flow veloeity of multiple blood vessels was decreased.Conclusions:The detection of BAEP and TCD is a sensitive index in detection of cerebral functional impairment of CMWP with hypoxemia,while the combination of these two detection would give us a better view of the changes of brain function.
