老年患者心脏起搏器植入术后生活质量的随访研究

Study on quality of life after implantation of pacemaker in aged patients

目的：评价老年患者心脏起搏器植入后的生活质量。方法：入选老年患者93例,年龄75~84,平均（78.54±3.05）岁,房室传导阻滞（AVB）且窦房结功能正常患者20例（21.5%）,病态窦房结综合征（SSS）患者45例（48.4%）,房颤（AF）患者24例（25.8%）,其他疾患4例（4.3%）。按照起搏器生活质量测定量表计分规则,于介入治疗前和治疗一年后计算生活质量得分,分值越低说明生活质量状况越好。结果：与治疗前比较,起搏器植入术后一年患者生活质量总评分明显降低[（89.95±2.53）分∶（86.12±1.61）分,P〈0.01],躯体功能、社会功能、心理功能以及一般认识均得到了不同程度的改善,其中以躯体功能改善最为明显[（31.45±2.41）分∶（28.63±1.45）分,P〈0.01]。影响老年患者生活质量的因素包括性别、有无配偶、心功能分级等（P均〈0.05）。结论：起搏器植入可明显改善老年患者的生活质量。

Objective： To evaluate quality of life after implantation of pacemaker in aged patients.Methods： A total of 93 aged patients were selected,aged 75~84 [average age（78.54±3.05） ]years,including 20 cases（21.5%） of atrioventricular block（AVB） with normal sinus node function,45 cases（48.4%） of sick sinus syndrome（SSS）,24 cases（25.8%） of atrial fibrillation（AF） and 4 cases（4.3%） of other heart diseases.According to the score rules of quality of life instrument for patients with pacemaker,scores of quality of life were calculated before and one year after pacemaker implantation,lower scores indicated a better quality of life.Results： Compared with before treatment,total scores of quality of life of the patients significantly decreased [（89.95±2.53） scores vs.（86.12±1.61） scores,P0.01],scores of physical function,social function,psychologic function and general recognition improved significantly,and improvement of physical function was most significant [（31.45±2.41） scores vs.（28.63±1.45） scores,P0.01].The factors that affected quality of life in aged patients included gender,possession of spouse,classification of cardiac function,P0.05 all.Conclusion： Implantation of pacemaker could significantly improve quality of life in aged patients.
