腹部皮瓣修复上肢复杂损伤26例

Effect of Repairing the Complex Defects of Upper Extremity by Using the Abdominal Skin flap

目的评价腹部皮瓣用于修复上肢复杂损伤的临床效果。方法 2005年12月～2010年7月对26例上肢复杂损伤实施腹部皮瓣修复,其中多指脱套伤11例采用单侧腹股沟皮管修复,手部掌背两侧复合脱套伤7例采用迟延皮瓣修复,上肢大面积撕脱伤8例采用脐上或侧胸壁至股部联合皮瓣修复,术后随访半年。结果 26例手术均顺利完成,皮瓣均成活,随访半年,皮瓣血运良好,质地柔软,上肢各关节活动无受限。结论腹部皮瓣可灵活应用于修复各种上肢复杂损伤,手术安全,效果可靠,值得临床推广。

Objective To evaluate the clinical effect of the abdominal skin flap for repairing the complex defects of upper extremity.Methods The abdominal flaps were used in 26 patients to repair the complex defects of upper extremity from December 2005 to July 2010.Among them,11 patients with degloving injury of the fingers were repaired with unilateral inguinal skin tubes,7 with complex degloving injury of the hand were repaired with delay flap,8 with large soft tissue defects of upper extremity were repaired with flaps above the umbilicus or chest wall.Results The 26 operations were all successful and all flaps survived.The patients were followed up for 6 months postoperatively.The flap was soft with good blood circulation,and motor functions of upper extremity Rom（range of motion） was similar to the normal side.Conclusion Repairing the complex defects of upper extremity by using the abdominal skin flap is safe,reliable and valuable in clinical application.
