全周和半周选择性激光小梁成形术治疗原发性开角型青光眼

Effect of 360 or 180 degrees selective laser trabeculoplasty on primary open angle glaucoma

目的比较应用全周和半周选择性激光小梁成形术治疗原发性开角型青光眼的疗效和并发症。方法原发性开角型青光眼126例(126只眼),分别采用532 nm倍频Q-开关Nd:YAG激光治疗仪,以光斑400μm,脉冲时间3 ns,能量0.4-0.8m J行全周和半周小梁成形术。观察治疗后眼压、视力、前房反应等。结果全周小梁成形术54例(54只眼)18个月时眼压平均下降(8.8±4.1)mm Hg,半周小梁成形术50例(50只眼)18个月时眼压平均下降(7.8±3.1)mm Hg,术后1 d和2个月时下降幅度最大。22例(22只眼)术后1 d及以后眼压仍超过30mm Hg者加用降眼压药物。术后雾视、前房反应均为一过性,视力均无变化。结论全周和半周选择性激光小梁成形术均可以持久有效地治疗原发性开角型青光眼。

Objective To compare the therapeutic effects of 360 degrees or 180 degrees selective laser trabeculoplasty（SLT） on eyes of primary open angle glaucoma（POAG）.Methods 126 cases 126 eyes of POAG were treated by SLT with light spot diameter of 400 μm,irradiation time of 3ns,and power of 0.4-0.8 mJ.Results Intraocular pressures（IOPs） of 54 cases 54 eyes decreased 8.8±4.1 mm Hg at the end of 18 months treated by 360 degrees SLT,while 50 cases 50 eyes decreased 7.8±3.1 mm Hg treated by 180 degrees SLT,with no change in visual acuity.Blurring and anterior uveitis were controlled.Conclusion 360 degrees or 180 degrees SLT may both be a safe,effective and prosperous therapy for POAG eyes.
