经尿道等离子前列腺电切术、睾丸切除联合持续性雄激素阻断治疗中晚期前列腺癌疗效观察

The effect observation of transurethral bipolar plasma kinetic prostatectomy and orchidectomy with continuity androgen deprivation in metaphase-advanced prostate cancer

目的观察经尿道等离子前列腺电切术(TUPKP)、睾丸切除术联合持续性雄激素阻断(CAD)治疗中晚期前列腺癌(PCa)的临床疗效。方法 30例中晚期PCa患者行TUPKP同时行双侧睾丸切除去势结合CAD治疗。观察治疗12个月后,患者国际前列腺症状评分(IPSS)、生活质量评分(QOL)、前列腺特异抗原(PSA)、最大尿流率(MFR)、残余尿量(RV)、前列腺体积等指标变化情况。结果治疗后12个月所有患者IPSS、QOL评分减少,PSA、RV水平及前列腺体积较治疗前均明显下降,MFR较治疗前明显增加,差异均有统计学意义(P<0.05)。结论 TUPKP、睾丸切除联合CAD治疗中晚期PCa的疗效显著,能显著提高患者的生活质量,值得临床推广应用。

Objective To investigate the effect of transurethral bipolar plasma kinetic prostatectomy（TUPKP）and orchidectomy combined with continuity androgen deprivation（CAD） in metaphase-advanced prostate cancer（PCa）.Methods 30 cases patients with PCa received the treatment of TUPKP and orchidectomy combine with CAD.After treatment 12 months,observed the situation of international prostate symptom score（IPSS）,quality of life assessment（QOL）,prostate specific antigen（PSA）,maximum urinary flow rate（MFR）,residual urine volume（RV）,prostate volume.Results After treatment 12 months,IPSS,QOL score of patients decreased,PSA,RV levels and prostate volume were significantly lower than before treatment,MFR was significantly increased compared with before,the differences were statistically significant（P0.05）.Conclusion TUPKP and orchidectomy combined with CAD in PCa has an obvious effect and it can improve the life quality.
