摘要
目的探讨食管癌、贲门癌术后早期肠内营养(EN)与肠外营养(PN)对患者术后恢复及并发症发生率的影响。方法将96例食管癌、贲门癌手术患者随机分成EN组和PN组,分别检测两组术前1天、术后第8天体重、血常规、血清白蛋白和转氨酶,并观察两组术后肛门排气时间、并发症发生情况并加以对照分析。结果 EN组患者体重、血红蛋白、血清白蛋白下降幅度均小于PN组(P<0.05),转氨酶上升幅度低于PN组(P<0.01)并发症发生率EN组明显低于PN组(P<0.01),作为免疫指标的淋巴细胞计数EN组较PN组恢复升高更快(P<0.01)。结论食管癌、贲门癌术后早期EN较PN能更好地改善患者营养状况,促进免疫功能恢复,并发症发生率更低,值得临床重视。
[Objective] To study the different effects of postoperative early enteral nutrition (EN) and parenteral nutrition (PN) on nutritional status, immune function and complication in patient with esophageal and cardiac carcinoma. [Methods] Ninety-six patients with esophageal and cardiac carcinoma were randomly divided into two groups. 46 cases with early EN and 50 cases with PN, the body weight, blood routine test, liver function, and complication on postoperative day 8 were compared with those before preparation. [Resuits] The body weight, the levels of hemoglobin, the albumin decreased less in EN group than those in PN group (P〈0.05). The alanine aminotransferase increased less in EN group than that in PN group (P〈0.01). The total complications occurred in EN group is much lower than that in PN group (P〈0.01). The blood lymphocyte in EN group is increased remarkable than that in PN group (P〈0.01). [Conclusions] EN can signifi- candy improved nutritional status, immune function, decrease the incidence of post-operative complications in patient with esophageal and cardiac carcinoma.
