重建钢板偏心性放置内固定术治疗锁骨中段骨折的临床应用

Clinical application of treatment of middle clavicular fractures with eccentrically placed reconstruction plate

目的探讨重建钢板偏心性放置内固定治疗锁骨中段骨折的疗效。方法选择2008年1月至2009年12月32例锁骨中段骨折的患者,进行重建钢板偏心性放置内固定术治疗,观察骨折愈合、并发症及肩关节功能恢复情况。结果 32例全部达到骨性愈合,未出现脱钉、钢板折弯、断裂等情况。按漂浮肩功能评分标准(Herscovici标准)进行功能分级,优22例,良7例,可3例,优良率为90.63。结论重建钢板偏心性放置加强了骨折远端的有效固定,可早期进行功能锻炼,是一种值得推荐的术式。

Objective To investigate the effect of middle clavicular fractures with eccentrically placed reconstruction plate and multi-angle,multi-directional of the screw.Methods Thirty-two patients with reconstruction plate placed eccentrially were followed up,then the fracture healing rate,complications and joint function recovery were analyzed.Results According to the standard floating shoulder function score（Herscovici Standard）,22 cases could be scored excellent,7 cases could be scored good,3 cases could be scored to be fair and none to be poor,the excellent rate was 90.63%.Conclusions The reconstruction plate placed eccentrically is the most effective treatment method for middle clavicular fractures.This approach enhances the intrinsic biomechanical factors and strengthens the fracture fixation.It is helpful for early physical exercise and merits wide application.
