韩氏穴位神经刺激仪辅助硬膜外麻醉用于急性阑尾炎手术的临床研究

CLINICAL OBSERVATION OF EPIDURAL ANESTHESIA COMBINED WITH HAN'S ACUPOINT NERVE STIMULATOR (HANS) FOR APPENDECTOMY

目的:观察普通外科急性阑尾炎病人在硬膜外麻醉时应用韩氏穴位神经刺激仪(HANS) ,观察是否能减轻内脏牵拉反应及相应的血糖变化。结果:硬膜外麻醉加HANS组及硬膜外麻醉对照组各15 例。+ HANS组有60 % (9/15) 的病人不出现牵拉反应,明显高于对照组13 .3 % (2/15) , 两组差异非常显著( P < 0 .01) ;+ HANS组术中需追加哌替啶的病例13 .3 % (2/15) ,显著少于对照组53 % (8/15)( P < 0 .01) ;+ HANS组血糖升高38 % ,显著低于对照组(54 % )( P < 0 .05) 。结论:硬膜外麻醉辅助HANS,可减轻手术牵拉反应及相应的升血糖反应。

Aim: To study whether epidural anesthesia can be strengthened by Han's Acupoint Nerve Stimulator. Methods: Thirty patients undergoing appendectomy were randomly devided into 2 groups: epidural guoup (control) and epidural combined with HANS (+HANS) group. The visceral traction reaction and the blood glucose levels during operation were observed. Results: No visceral traction reaction was observed in 60% (9/15) of the patients in +HANS group whereas only 13.3% (2/15) in the control group (P<0.01). Significantly less patients (13.3%, 2/15) needed additional pethidine injection in the +HANS group, as compared to the control group (53%, 8/15)(P<0.01). In the control group there was a 54% increase in blood glucose compared with baseline level, whereas in +HANS group the increase was only 39% (P<0.05). Conclusion: Epidural anesthesia combined with HANS was more effective in reducing visceral traction reaction and preventing the increase in blood glucose lever than epidural alone in patients undergoing appendectomy.
