强化血糖控制对外科手术的影响(60例胸科手术的分析)

目的:了解强化血糖控制对合并糖尿病的胸外科手术的影响。方法:根据糖化血红蛋白水平将60例糖尿病需要胸外科手术的患者分成3组:6%～8%组、8%～10%组和>10%组。选择同时期无糖尿病的胸科手术患者作为对照组,分析各组手术后常见并发症的差别。结果:与对照组相比,糖尿病胸外科手术组术前血糖越高,围手术期越长;手术并发症与血糖水平有关。糖尿病患者中,术前糖化血红蛋白<8%组,手术后并发症均明显低于糖化血红蛋白高于>10%组,且差异具有统计学意义(P<0.05)。结论:对于行胸部手术的糖尿病患者,加强血糖控制,平稳降糖才是保证手术效果和防止手术后并发症的关键。

Objective ：To investigate the effect of intensified glucose control on prognosis and complications in diabetes patients with thoracic surgery. Methods：60 diabetic cases were divided into 3 groups according to HbA1c,6% - 8% group, 8% - 10% group and above 10% group. The others, without diabetes also suffered from thoracic surgery at the same time, were the control group. To analysis the difference of complications among the groups. Results： Comparing with control group, the higher fasting blood glucose,the longer perioperative period in diabetic group. The complications were relative to fasting blood glucose. Conclusion：To intensify monitoring and control blood glucose in diabetic patients with thoracic surgery is very important for assuring the effectiveness and preventing complications.
