老年喉癌病人的喉功能保全及重建手术

CONSERVATION SURGERY AND VOICE RECONSTRUCTION IN SENILE PATIENTS WITH LARYNGEAL CARCINOMA

①目的 探讨喉功能保全手术及全喉切除喉发音重建手术在老年喉癌病人治疗中的应用。②方法对５２ 例６５ 岁以上的喉癌病人进行回顾性分析，其中声带切除术６ 例，垂直喉部分切除术２７ 例，声门上水平喉部分切除术２ 例，喉次全切除术８ 例，全喉切除发音重建术９ 例。③结果 行喉部分切除术的４３ 例均恢复发声功能和吞咽保护功能，拔管３７ 例；行全喉切除发音重建术的９例中恢复发声功能者７ 例；５年生存率为６５．５２％ ．无严重并发症发生，无术中及术后死亡。④结论 在适当掌握适应证，充分做好围手术期处理的条件下，老年喉癌病人的喉功能保全及重建手术是可行的。

Objective\ To study the application of conservation surgery and voice reconstruction in senile patients with laryngeal carcinoma.\ Methods\ Fifty two patients with laryngeal carcinoma who were above 65 years old and underwent conservation surgery or voice reconstruction were studied retrospectively. Among them, 6 cases underwent vocal cord resection, 27 vertical hemilaryngectomy, 2 horizontal supraglottic hemilaryngectomy, 8 subtotal laryngectomy, 9 total laryngectomy with voice reconstruction.\ Results\ All of the 43 patients underwent partial laryngectomy had their phonation and swallowing retored. Seven of 9 patients with total laryngectomy with voice reconstruction had their phonation restored. The 5 year survival rates were 65.52% and there were no severe complication.\ Conclution\ The conservation surgery and voice reconstruction in senile patients was feasible in senile patients with correct indication and thoroughly care.\;
